According to the Defense POW/MIA Accounting Agency (DPAA) website at the below link, the remains of former MIA Colonel David Hartzer Zook, Jr., USAF, were accounted for on 20 August 2008:

The latest DPAA update (15 January 2021) lists 1,053 Americans accounted for in Southeast Asia from the Vietnam War. Col. Zook is one of those 1,053 Americans (513 from the U.S. Air Force).

Former MIA Colonel David H. Zook, USAF - - SEA FIXED-WING AIR LOSS

Information about the mid-air collision and crash of a U-10B Courier prop aircraft piloted by former MIA Major David Hartzer Zook, Jr., USAF, with a C-7A Caribou in Southeast Asia (SEA) (over South Vietnam) on 4 October 1967 is available at this link/URL on the Vietnam Air Losses website:
