On the date of his loss, 16 November 1965, Captain Green was the pilot of an F-105D Thunderchief jet aircraft assigned to the 469th Tactical Fighter Squadron on a combat mission over North Vietnam. Following his attack on the target, his F-105 was hit by fragments of a surface-to-air missile (SAM). Capt. Green radioed other pilots in his flight that he planned to fly his injured aircraft to the Gulf of Tonkin, then eject near a U.S. destroyer. Although he made it to the Gulf, his F-105 broke in half and disintegrated before he was able to bail out.
