Crash Information on U.S. Army helicopter OH-23G tail number 64-15180
While climbing to 180 feet, a grinding noise was heard and the RPM started to bleed off. The engine quit and the transmission light came on. While making an autorotation into jungle growth, the left skid struck a log and the aircraft rolled on its right side. Later, during a rescue attempt from a similar aircraft, the rotor blades of the rescue aircraft (OH-23G 63-12784) struck observer SMAJ Cyril E. Stirnkorb as he stood on the skid of the downed aircraft, causing fatal injuries. The pilot of 64-15180 was rescued. The suspected causes of the crashes were transmission andor engine failure on 64-15180 and engine failure on the rescue aircraft. [Taken from vhpa.org]
