In Memory Application 2021
The 2021 deadline for submission is March 5, 2021.
VVMF’s In Memory program honors those who returned home from Vietnam and later died. Please understand that while your loved one’s memory and service will be honored, his/her name will not be inscribed on The Wall.
To have a loved one considered for the In Memory program in 2021, you must submit your application to VVMF by March 5, 2021. Please fill out all of the required information below.
Along with the application we require:
- Copy of military documents that establish service in the Vietnam war zone (e.g. DD214, service records, Vietnam Service Medal, or other documents/awards)
- You can request military documents here: http://www.archives.gov/veterans/
- Copy of death certificate or other formal notification of death
- Two clear photographs of the honoree (we recommend one photo from the Vietnam era and one photo that is more recent)
An optional biography may also be submitted. The biography should be no more than 300 words. Please avoid handwritten bios, if possible. If the biography is longer than 300 words, VVMF may edit for length. The biography may include any information which you are comfortable with sharing.
PLEASE DO NOT PRINT AND MAIL THIS PAGE. YOU CAN FIND THE PRINTABLE APPLICATION HERE.
